Initial Consultation - 50 minutes
Your initial appointment will begin with an assessment of your current nutritional status, habits and needs based on information you provide. We will discuss your personal nutrition goals & questions and, together, develop a nutrition plan that will work for your lifestyle.
Call for a complimentary 15 minute phone consultation.
Follow-up Sessions - 30 minutes
During follow-up appointments we will review your progress, whether it's weight loss, eating for more energy or gaining control of your blood sugar. We will revisit your goals and discuss ways that will enable you to find your optimal level of health.
Regular follow-up sessions are highly recommended for nutrition counseling to be most successful.
If you are currently under the active care of a physician for a medical condition for which you are interested in managing through medical nutrition therapy, a physician’s referral is required. Please contact Sarah for a referral form prior to scheduling an appointment.
I am a Medicare Part B provider and can provide Medical Nutrition Therapy for clients with a diagnosis of diabetes and/or renal disease. A physician's referral is required.
